PAYMENT
Full payment is due at the time of service for both insurance and non-insurance patients. For insurance patients, you will be reimbursed upon payment from the insurance company.

INSURANCE
I ACCEPT:
- AUTOMOBILE ACCIDENT INSURANCE
- HEALTH INSURANCE
However, I do not participate in any HMO's or PPO's. If you are insured by an HMO or PPO, you may still receive coverage, but at a reduced rate. Please contact your insurance company for further information.
I DO NOT ACCEPT:
- MEDICARE/MEDICARE SUPPLEMENTAL
You can still receive treatment if you are Medicare eligible, but it would have to be for maintenance or wellness care which is not covered by Medicare or Medicare Supplemental. In other words, you would be paying for the treatment yourself.
- MEDICAID
(TITLE 19/BADGER/FORWARD/HIRSP)
If you have this type of insurance, you can still receive treatment...however, it would be on a self-pay basis.
- WORKER'S COMPENSATION
If you have a Worker's Compensation injury, you can still receive treatment...however, it would be on a self-pay basis.
